Create Health performs a structured approach to improving patient flow in two phases: initially we Analyse the existing pathway for each gate (entry points, delays, capacity), then Engage key representatives from the health service to identify opportunities.

Next, we collaboratively Design interventions and solutions with key stakeholders, utilising our design thinking model to devise a comprehensive implementation roadmap detailing recommendations for implementation. Phase 2 entails Implementation by intervention and gate adopting implementation and behavioural science to enable change that sticks, and finally Evaluation by re‑mapping the original processes and measuring impact.

Phase One

Analyse

Identify the service points of entry and volumes. Identify time to complete Stages in each part of the patient journey and the barriers to proceed to the next Stage.

Engage

Confirm with the clinical services the volumes and performance of the clinical pathways. Confirm opportunities for improvement.

Co-design Interventions

Develop the models and processes for implementation of the selected initiatives.

Phase Two

Implement

Optimal impact is achieved through staging improvements that can be realised by providing capacity for the preceding gates.

Evaluation and Benefits Realisation

Perform process mapping exercises (Phase 1) again to determine success of applied interventions.

Industry Experience

Our team has worked with the community-controlled and primary care sectors to design solutions between primary/community care and the acute system. The team of clinicians and operational staff have also delivered the design and implementation of patient flow solutions into complex acute environments across more than 10 health networks and at a state-wide level. A strong focus of this work includes the transfer of care between providers and the community.
